Why a row per night instead of one summary?

A stay is several days, and the detail that matters is often one of them: the night dinner was left, or the morning a dose was late. One row per day shows that at a glance and gives you a record if an owner asks.

Who should initial the medication column?

The person who gave the dose, each time they give it. Initials make it clear the dose was given and by whom, which matters at shift change.

Does this replace a boarding agreement?

No. This card records what happened during the stay. Your terms and the owner’s consent belong in your own agreement, which a lawyer should review. The policy builder on this site gives you a starting point.

Should the report card go home on paper or by text?

Either works. Paper suits a pick-up at the counter. A text suits an owner who collects after hours or never comes in, because the card reaches them wherever they are. If you print it, keep a photo of the filled-in card on the pet record so the next team member can see it.

Boarding Report Card Template

This card records a whole stay, one row per night: breakfast, dinner, medication, toilet breaks, exercise and sleep. At pick-up it doubles as the belongings check, so the lead, the bed and the leftover food go home with the pet.

Use the printout

At check-in, fill in The stay and tick every item the owner left with you.
Fill in one row of the log each day. Initial the medication column each time a dose is given.
At check-out, tick the belongings again as they go back to the owner.
Write a short note on how the stay went and hand the card over.

In the print dialog, choose Save as PDF to keep a copy.
