Which vaccinations should I record?

The ones your business requires, as named on the certificate. Requirements differ by country and by service, and group care usually asks for more than a one-to-one visit. Your own policy decides the list; your local vet can advise on it.

Why log every dose instead of ticking the schedule?

A schedule says what should happen. A log says what did, at what time and from whom. When a pet stays with you for several days, or the team changes over, the log is the record you rely on.

What if the owner’s instructions and the label differ?

Follow the label and call the owner before the next dose. Write the call and the answer in the Notes column.

Is this medical advice?

No. It is a record of what the owner and their vet have told you and what your team has given. Questions about a dose or a reaction go to the pet’s vet.

Pet Vaccination and Medication Record Template

This record keeps one pet’s vaccinations, current medication and a log of every dose given on one page. It shows at a glance whether a vaccination has lapsed before the pet joins a group, and who gave each dose.

Use the printout

Fill in Vaccinations from the certificate, and initial Certificate seen once you have checked it.
Fill in Current medication from the owner’s instructions or the vet’s label.
Each time a dose is given, write a row in Doses given and initial it.
Check the Due again dates before each visit.

In the print dialog, choose Save as PDF to keep a copy.
